HOSPITAL DISCHARGE PROCEDURES (§ 32.1-137.02)

Before a hospital discharges a patient, it shall, to the extent allowed pursuant
to state and federal law, inform and educate the patient, and his family when it
is involved in decision making or ongoing care, about his follow-up care,
treatment, and services.
