and\n\n12\n\nIn the case of a therapeutic radiopharmaceutical, the patient’s name.\n\nC\n\nOrders for radiopharmaceuticals, whether written or verbal, shall include at least the following information:\n\n1\n\nThe name of the institution or facility and the name of the person transmitting the order;\n\n2\n\nThe date that the radiopharmaceutical will be needed and the calibration time;\n\n3\n\nThe name or generally recognized and accepted abbreviation of the radiopharmaceutical;\n\n4\n\nThe dose or activity of the radiopharmaceutical at the time of calibration; A.<\/span> A pharmacist who is authorized by the Board<\/span> and acting in good faith, may sell and dispense<\/span> radiopharmaceuticals<\/span> pursuant to the order<\/span> of a physician who is authorized by state or federal law<\/span> to possess and administer<\/span> radiopharmaceuticals<\/span> for the treatment or diagnosis of disease. <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

B.<\/span> When an authorized nuclear pharmacist dispenses<\/span> a radioactive medical material<\/span>, he shall assure that the outer container (shield) of the radiopharmaceutical<\/span> shall bear the following information: <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

1.<\/span> The name and address of the nuclear pharmacy; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

2.<\/span> The name of the prescriber<\/span> (authorized user); <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

3.<\/span> The date of dispensing<\/span>; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

4.<\/span> The serial number assigned to the radiopharmaceutical<\/span> order<\/span>; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

5.<\/span> The standard radiation symbol; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

6.<\/span> The name of the diagnostic procedure; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

7.<\/span> The words “Caution: Radioactive Material<\/span>”; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

8.<\/span> The name of the radionuclide; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

9.<\/span> The amount of radioactivity and the calibration date and time; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

10.<\/span> The expiration date and time; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

11.<\/span> In the case of a diagnostic radiopharmaceutical<\/span>, the patient’s name or the words “Per Physician’s Order<\/span>”; and <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

12.<\/span> In the case of a therapeutic radiopharmaceutical<\/span>, the patient’s name. <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

C.<\/span> Orders<\/span> for radiopharmaceuticals<\/span>, whether written or verbal, shall include at least the following information: <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

1.<\/span> The name of the institution or facility and the name of the person<\/span> transmitting the order<\/span>; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

2.<\/span> The date that the radiopharmaceutical<\/span> will be needed and the calibration time; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

3.<\/span> The name or generally recognized and accepted abbreviation of the radiopharmaceutical<\/span>; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

4.<\/span> The dose or activity of the radiopharmaceutical<\/span> at the time of calibration; and <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t
