The election authorized by § 15.2-3505 shall be conducted in accordance with general law. The ballots used shall be printed and shall contain the following:

“Shall ____________________ (here insert names of counties, cities or towns proposing to consolidate) consolidate pursuant to the consolidation agreement?

[ ] Yes

[ ] No.”
